2) Form 730;
3) UNICO income individuals.

• You can have only one choice of destination
• The choice in favor of 5 per thousand does not replace that of 8 per thousand, which remains in effect but adds to it
• The choice of 5 per thousand does not involve an increase of taxes to be paid by the
contribuente
